PAYMENT AND INSURANCE INFORMATION

• Mind Body Connection Health and Wellness currently accepts Aetna, Blue Cross Blue Shield, Cigna, Optum, United Healthcare, UMR, Oscar, Harvard Pilgrim, Oxford, UHC Student Resources, All Savers (UHC), Health Plans Inc, Bind, Lyra, Meritain, Nippon, Allied Benefit Systems, GEHA - United Healthcare Shared Services (UHSS). 
•  Payments can also be accepted by Debit/HSA/Credit Card, due at the time of service.
• Mind Body Connection Health and Wellness can provide you with a billing statement, for out of network services which you can submit to your insurance company for possible reimbursement. Reimbursement is not always guaranteed, but sometimes insurance companies reimburse the patient up to 60% of the visit for out of network services.

If you have an Out Of Network PPO insurance plan and have met your deductible, it is likely that a percentage of the charges will be reimbursed to you at the out-of-network rate. In addition, services may apply towards qualified medical expenses if you have a Health Care Reimbursement Account (HCRA) or a Health Savings Account (HSA) through your employer.

We will provide an itemized superbill that includes appropriate ICD (medical diagnostic codes) and CPT (procedure codes) for you to submit to your insurance for reimbursement after you have made a payment to our office.
